Job summary

The Hampton-Newport News Community Services Board is seeking a Care Coordinator to join our Regional Crisis Stabilization Center. You will help individuals transition from crisis stabilization to the community by coordinating care and connecting them with behavioral health, medical, and social resources.

You will collaborate with a multidisciplinary team to assess needs, develop service plans, document in electronic health records, and promote continuity of care while maintaining confidentiality.

Qualifications

• Master's degree in human services required.
• Minimum 1 year experience in service coordination and psychoeducation.
• QMHP certification or trainee registration within 30 days of employment.

Responsibilities

• Coordinate referrals, discharge planning, aftercare and transitions of care.
• Connect individuals with behavioral health, medical, social, and community resources.
• Provide case management and benefits assistance.
• Develop individualized service plans and maintain timely electronic health record documentation.
• Collaborate with internal teams and community providers to promote continuity of care.
• Review pre-screening evaluations to determine referral appropriateness for crisis stabilization.
• Provide direct support and crisis intervention while maintaining confidentiality.
