Job summary

Qualifications

• One year of healthcare setting experience preferred.
• Current National Medical Assistant Certification required.
• Current Idaho Practical Nursing License or equivalent required.
• Current BLS certification.

Responsibilities

• Telephone triage for patient concerns.
• Perform patient follow-up and coordinate care per protocol.
• Refill patient medications per protocol.
• Enter and track patient recalls (immunizations, tests, screenings).
• Track outpatient referrals per policy and procedure.
• Coordinate with other departments to ensure timely patient flow.
• Act as float to support in-person clinical care.
• Participate in meetings and quality improvement activities.
• Assist with outreach events and document outcomes.
• Coach patients on chronic disease self-management.
