Job summary

Qualifications

• High School Diploma or equivalent.
• Graduation from a Medical Assistant Program or Equivalent.
• Graduation from a Practical Nursing Program or Equivalent.
• Current National Medical Assistant Certification.
• Current BLS certification.

Responsibilities

• Provide telephone triage for patient concerns and questions.
• Perform patient follow-up and coordinate care per clinic protocol.
• Refill medications per clinic protocol.
• Enter recalls and track patient reminders (immunizations, tests, screenings).
• Coordinate outpatient referrals and track follow-ups.
• Act as float to cover staff shortages in clinical nursing.
• Participate in meetings and quality improvement activities.
• Assist with outreach events and data collection for reporting.

Telephone triage for patient concerns/questionsPerform patient follow-up and coordinates care (e.g. obtain prior authorizations, follow-up on abnormal results, manage appropriate buckets in EMR, etc.), per clinic protocolRefill patient medications per clinic protocolEnter in and tracks patient recalls as appropriate (childhood and adult immunizations, future blood work, abnormal paps, colonoscopies, etc.)Track outpatient referrals, per policy and procedureReview and coordinate with other departments and integrate work with other staff to ensure timely and accurate patient flowAct as float to help fill in during staff shortages for in-person patient care in the clinical setting; follow workflows, policy procedures set for clinical nursingParticipate in meetingsParticipate in continuous quality improvement activitiesAssist with outreach activities such as health fairs, when neededDocument client encounters from outreach events, activities, service plans, and outcomes achieved by client in an effective manner; completes and submits monthly reports; and ensures accurate data collection for quality measures and other reporting purposeCoach patients in effective management of their chronic health conditions and self-care. Assist patient in understanding care plans and instructions.
