Qualifications

Responsibilities

  • Telephone triage for patient concerns/questions.
  • Perform patient follow-up and coordinate care (e.g., prior authorizations, abnormal results, EMR follow-up).
  • Refill patient medications per clinic protocol.
  • Enter and track patient recalls (immunizations, tests, pap smears, colonoscopies).
  • Track outpatient referrals and coordinate per policy.
  • Coordinate with other departments to ensure timely patient flow.
  • Act as float to fill in during staff shortages for in-person care.
  • Participate in meetings and continuous quality improvement.
  • Assist with outreach activities and health fairs as needed.
  • Document client encounters from outreach events and ensure data accuracy for reports.
  • Coach patients on managing chronic conditions and understanding care plans.
  • Assist patients in accessing health services and overcoming barriers.
  • Comply with Health West Inc. policies and procedures.
  • Other duties as assigned.

Perform patient follow-up and coordinates care (e.g. obtain prior authorizations, follow-up on abnormal results, manage appropriate buckets in EMR, etc.), per clinic protocol3. Refill patient medications per clinic protocol4. Enter in and tracks patient recalls as appropriate (childhood and adult immunizations, future blood work, abnormal paps, colonoscopies, etc.)5. Track outpatient referrals, per policy and procedure6. Review and coordinate with other departments and integrate work with other staff to ensure timely and accurate patient flow7. Act as float to help fill in during staff shortages for in-person patient care in the clinical setting; follow workflows, policy procedures set for clinical nursing8. Participate in meetings9. Participate in continuous quality improvement activities10. Assist with outreach activities such as health fairs, when needed11. Document client encounters from outreach events, activities, service plans, and outcomes achieved by client in an effective manner; completes and submits monthly reports; and ensures accurate data collection for quality measures and other reporting purpose12. Coach patients in effective management of their chronic health conditions and self-care. Assist patient in understanding care plans and instructions. Motivate patients/clients to be active and engage event participants in their health and overall wellbeing.13. Assist patients in accessing health-related services to appropriate medical and specialty providers and overcoming barriers to obtaining needed medical care and /or social service14.
