Job summary

OHSU in East Portland seeks a Care Coordinator to support the clinic team within the Patient Centered Primary Care Home model. You will manage non-nurse aspects of patient care, coordinate labs and scheduling, and communicate with patients through EPIC and MyChart.

The role emphasizes independent work, strong communication, and teamwork with LIPs and care teams. Daytime hours, on-site at East Portland clinic, with focus on high-quality patient service.

Qualifications

• Two years’ experience in a hospital or hospital-based clinic setting.
• High School diploma or equivalent.
• MA certification received from a nationally recognized certifying body by hire or probation period.
• Current Oregon EMT license or Oregon Practical Nurse license is acceptable under qualification options.

Responsibilities

• Assist provider with order entry, medication refills, and patient education.
• Coordinate labs and scheduling orders per care plan; communicate with patients.
• Monitor EPIC inbox for messages and refill requests requiring attention.
• Scrub charts daily; proactively schedule appointments; handle referrals and tests.
