Job summary

The Centers in Little Rock, AR is seeking a Care Coordinator to support clients across mental health services. You will educate clients about their condition, monitor progress, and assist the care team in planning and implementing interventions to promote wellness.

This safety-sensitive role requires a BA in Psychology or related field, 3+ years in care coordination or clinical practice, strong Microsoft Office skills, and the ability to travel with a valid Arkansas driver's license.

Qualifications

• BA degree in Psychology or related field.
• At least 3 years of care coordination or clinical practice experience.
• Must be able to travel and hold a valid Arkansas driver’s license.
• Computer proficiency including Microsoft Office.
• Safety-sensitive position.

Responsibilities

• Educate clients about their condition, medication, and support medication adherence.
• Check-in on the client regularly and evaluate and document their progress.
• Assist the care team with developing and implementing interventions.
• Provide discharge planning.
• Help client adhere to legal responsibilities.
• Assist client to obtain and maintain stable housing.
• Assist the treatment team to develop a care plan to address the client’s mental health care needs.
