Qualifications

• Certified BH case manager II or equivalent experience.
• Bachelor's/Master’s BH degree or RN with BH experience.
• Ability to complete trainings and maintain paperwork on time.

Responsibilities

• Coordinates services including mental and physical health.
• Performs multi-level assessments within set timelines.
• Provides crisis assistance and advocacy for families.
• Maintains relationships with community agencies and stakeholders.
• Participates in staff meetings and trainings.
