Sirona Recovery is seeking a Care Coordinator to support Comprehensive Community Services (CCS) for adults with mental illness and/or substance use disorders. The role focuses on helping individuals understand, navigate, and grow through the CCS program with a strengths-based, person-centered approach.

The coordinator will collaborate with providers, families, and community teams, ensuring recovery plans are implemented and adjusted as needed.

Competenze

• Bachelor’s Degree in Social Work or related field; Master’s Degree preferred.
• At least one year of experience working in the mental health and/or substance use fields preferred.
• Pass a pre-employment background check.
• Valid driver’s license and automobile with insurance.

Mansioni

• Help consumers understand, navigate, and grow through the CCS program.
• Meet with the consumer’s service providers and other team supports.
• Assist in developing the recovery plan of care.
• Receive consultation from mental health and substance use professionals while working on recovery plans.
