Job summary

Sirona Recovery seeks a Care Coordinator to operate within the Comprehensive Community Services (CCS) program. You will help adults with complex needs navigate care plans, collaborate with providers, and support recovery goals in a compassionate, trauma-informed framework.

The position emphasizes person-centered care, interdisciplinary teamwork, and licensure-oriented supervision. A hybrid work model and ongoing training opportunities support professional growth and licensure requirements.

Qualifications

• Bachelor’s Degree in Social Work or related field; Master’s Degree preferred.
• Experience in mental health and/or substance abuse fields preferred.
• Valid driver’s license and automobile with insurance.
• Pre-employment background check and TB testing and flu vaccinations as required.
• Ability to work with diverse populations and LGBTQ+ identities.

Responsibilities

• Assist consumers in understanding, navigating, and engaging with the CCS program.
• Coordinate with service providers and support teams to implement recovery plans.
• Provide supervision and training resources; support licensure processes.
• Maintain high-quality CCS services with trauma-informed, person-centered care.
