Job summary

Intermountain Centers is seeking a Care Coordinator to manage a member’s whole health using an integrated case management model. The role includes assessment, planning, implementation, and monitoring within the Electronic Health Record system.

The position requires a bachelor’s degree in a related field or an associate’s with relevant experience, and/or a high school diploma with substantial behavioral health experience. Strong documentation and coordination skills are essential.

Qualifications

• Bachelor’s degree in a field related to behavioral health or social work.
• Associate’s degree in a related field with 2+ years of related experience.
• High school diploma with 4+ years behavioral health related experience.

Responsibilities

• Observe members, listen to concerns, and record observations in clinical records.
• Document and report concerns related to member behavior/interactions.
• Monitor health and safety of members and report concerns per policies.
• Maintain member records, assessments, and treatment plans.
• Develop time-specific, action-oriented individual service plans.
• Complete progress notes using billing codes and meet engagement expectations.
• Follow HIPAA policies and maintain PHI securely.
• Possess ability to transport members in approved conditions as needed.

Education

Bachelor’s degree in behavioral health or social work

Associate’s degree in behavioral health or social work with 2+ years related experience

High School diploma with 4+ years behavioral health related experience
