Job summary

Intermountain Centers is seeking a Care Coordinator to manage an integrated case management approach, including assessment, planning, implementation and monitoring of member health services. Qualifications

• Bachelor’s degree in a field related to behavioral health or equivalent.
• Associate’s degree plus 2 years related experience as alternative.
• High school diploma with 4 years behavioral health experience.

Responsibilities

• Observe members, listen to concerns, and record observations.
• Document and report concerns in clinical records per policy.
• Monitor health and safety of members and report concerns.
• Maintain member records, assessments and treatment plans.
• Collaborate with inter-disciplinary teams to improve member care.
• Serve as advocate and liaison to access community resources.
