Job summary

The role requires a Bachelor's in Social Work or related field, with preferred experience in behavioral health and collaboration across providers.

Summary:
The Care Coordinator - Behavioral Health role provides direct outreach and linkages to community resources to consumers/clients in assigned programs. Guides clients, consumers and their families through the care continuum establishing contact upon referral and continuing contact throughout care and to self- management and/or active treatment.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Bachelor's Degree in Social Work or related field preferred.
2. License/Certifications: N/A
3. Experience: Two years of experience working with behavioral health consumers and families preferred. Experience in a team based setting and collaborating with multiple providers preferred.

The pay range for this position is $52,499.20to $71,052.80per year.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
