Job summary

DayToDay Health is seeking a Care Coordinator to support children aged 0–3 with developmental delays through the Birth to Three Program. You will assess needs, develop and implement family-centered service plans, coordinate services, advocate for families, and maintain documentation in collaboration with community providers.

This community-based role involves travel to support diverse families, with a strong emphasis on trauma-informed, respectful service delivery and teamwork.

Qualifications

• Bachelor's degree in Social Work, Child Development, or related field.
• Experience working with families with special needs preferred.
• Valid driver's license and reliable transportation required.
• Strong communication and organizational skills; ability to work in varied settings.
