Job summary

Cherry Hill Free Clinic seeks a Care Coordinator to support children aged 0-3 with developmental delays or disabilities and their families through the Birth to Three Program. You will conduct assessments, develop family-centered service plans, coordinate services, and facilitate team meetings.

You will partner with families, service providers, and community resources to promote development, provide education and advocacy, and ensure access to needed supports.

Qualifications

• Bachelor's degree in Social Work, Child Development, or related fields.
• Strong documentation, communication, and organizational abilities; comfort working in diverse settings.
• Preferred experience with families of children with special needs.
