Rock Health is seeking a Care Coordinator for the Birth to Three Program. The role involves partnering with families, service providers, and community resources to support children aged 0–3 with developmental delays or disabilities.
You will conduct assessments, develop family-centered plans, coordinate services, and facilitate team meetings. Responsibilities include education, advocacy, and maintaining compliant documentation while collaborating with internal and external teams.
Role: Care Coordinator for Birth to Three Program
Responsibilities: Partner with families, service providers, and community resources to support children aged 0–3 with developmental delays or disabilities. Conduct assessments, develop and implement family-centered service plans, coordinate services, and facilitate team meetings. Provide education, advocacy, and maintain accurate documentation in compliance with regulations. Collaborate internally and externally, managing a flexible schedule to meet client needs, including occasional evening or weekend hours. Ensure timely service delivery and meet productivity expectations.
Qualifications: Bachelor's degree in Social Work, Child Development, or related field; preferred experience working with families with special needs. Valid driver's license, reliable transportation, and strong organizational, communication, and documentation skills are required. Ability to work in various settings and respond appropriately to crises is essential.
Work Environment: Community-based role involving daily travel, physical activity, and exposure to diverse environments in homes and communities. Travel may include occasional overnight stays.
