Job summary

The VIOS Clinic is seeking a Care Coordinator for the Birth to Three Program, an Early Intervention initiative serving children aged 0–3 and their families.

In this community-based role, you will partner with families, service providers, and community resources, conduct functional assessments, and develop family-centered service plans. You will coordinate services across care teams, provide education and advocacy, and maintain compliant documentation.

Qualifications

• Bachelor's degree in Social Work, Child Development, or related human services field.
• Preferred: at least one year of supervised experience working with families with special needs.
• Valid driver's license, reliable transportation, and ability to meet driving and insurance requirements.
• Strong documentation, communication, and organizational skills.
• Comfort working in home, office, and community settings.
• Ability to respond appropriately in crisis situations.
