Job summary

Black Women's Mental Health Institute in Wisconsin seeks a Care Coordinator for the Birth to Three Program to support children aged 0–3 with developmental delays or disabilities and their families. You will partner with families and providers to create family-centered service plans, conduct assessments, coordinate services, and advocate for resources through home visits and community settings.

This community-based role requires strong organization and crisis-response skills.

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related human services field.
  • Experience working with families with special needs preferred.
  • Valid driver's license, reliable transportation, and ability to work in diverse settings.
  • Strong organizational, communication, and documentation skills.
  • Ability to respond effectively in crisis situations.
