Job summary

Apex Home Health, Hospice & Home Care is seeking a Care Coordinator for its Birth to Three Program in Wisconsin. You will partner with families, service providers, and community resources to promote child development, conduct functional assessments, and develop individualized, family-centered service plans for children aged 0–3.

The role coordinates, facilitates, and monitors services, provides education and advocacy to families, maintains documentation, and participates in family-centered team

Qualifications

• Bachelor's degree in Social Work, Child Development, or related field.
• Strong communication and documentation abilities.
• Ability to travel daily in community settings and respond to crises.

Job description

Role: Care Coordinator for the Birth to Three Program, supporting children aged 0–3 with developmental delays or disabilities and their families.

Responsibilities

• Partner with families, service providers, and community resources to promote child development.

• Coordinate, facilitate, and monitor services aligned with care plans.
• Provide education, advocacy, and support to families.
• Maintain accurate documentation and facilitate family-centered team meetings.

• Manage a flexible schedule, including occasional evenings or weekends.
• Meet productivity and service delivery expectations.

Qualifications

• Bachelor's degree in Social Work, Child Development, or related field.
• Preferred: at least one year of supervised experience working with families with special needs.
• Valid driver's license, reliable transportation, and ability to meet driving and insurance requirements.
• Strong organizational, communication, and documentation skills.
• Comfort in home, office, and community settings; able to respond appropriately in crises.

Work Environment & Skills: Community-based with daily travel, moderate physical activity, and exposure to diverse environments; requires adaptability and organizational skills.
