Job summary

The VIOS Clinic is seeking a Care Coordinator for the Birth to Three Program in Milwaukee, WI. You will support children ages 0–3 with developmental delays or disabilities and collaborate with families and community resources.

This role involves conducting assessments, developing individualized service plans, and coordinating multidisciplinary services. Daily travel to homes and community settings is expected, with occasional evenings or weekends.

Qualifications

• Bachelor's degree in Social Work, Child Development, or related field.
• Preferred: one year of supervised experience with families with special needs.
• Valid driver's license and reliable transportation.
• Strong communication, organizational, and documentation skills.
• Comfort working in home, office, and community settings.
• Ability to respond effectively in crisis situations.
