Job summary

Omni Family of Services Tennessee Inc is seeking a Care Coordinator to meet with clients and families in homes or community settings, coordinating care and assessments. Hours vary but typically stay within a 40-hour workweek, with on-call availability.

We emphasize safety, trauma-informed care, and supportive teamwork. A bachelor’s degree in a behavioral health field and valid driver’s license are required, with comprehensive benefits and ESOP ownership.

Qualifications

• Bachelor’s degree in a behavioral health field is preferred.
• Valid driver’s license and proof of automobile insurance are required.
• Certification in crisis prevention initiatives and universal precautions is required.
• Ability to meet clients and families in home or community settings and coordinate care.

Responsibilities

• Meet with client(s) and families in home or residence for care coordination and assessments.
• Coordinate services and advocate for client needs across settings (school, court, job site, etc.).
• Be available for on-call and after-hours support as needed.

The Care Coordinator generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis with Phone call triage assessments during the off months and weeks of face to face care. In addition, the Care Coordinator may meet with the members of the client’s extended family and community supports at locations such as school, court, job site, etc., as specified to meet the client’s service and advocacy needs. The Care Coordinator’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week.
