Job summary

Omni Family of Services Tennessee Inc is seeking a Care Coordinator to meet with clients and families in their homes or residences, providing regular, personalized support. The role includes coordinating assessments and connecting clients to services within foster care, adoption, and community-based supports.

Work hours vary and can include on-call and after-hours, with a typical cap of 40 hours per week. You will collaborate with families, schools, and community partners to ensure safety,

Qualifications

• Bachelor’s degree in behavioral health or related field required.
• Valid driver’s license and insurance.
• Crisis prevention and universal precautions certification required.

Responsibilities

• Meet with client and family in home or residence on a regular schedule.
• Provide phone triage assessments during off months and weeks of face-to-face care.
• Coordinate with client’s extended family and community supports as needed.

The Care Coordinator generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis with Phone call triage assessments during the off months and weeks of face to face care. In addition, the Care Coordinator may meet with the members of the client’s extended family and community supports at locations such as school, court, job site, etc., as specified to meet the client’s service and advocacy needs. The Care Coordinator’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week. • A minimum of a bachelor’s degree required, with said degree being in a behavioral health related field such as counseling, social work, or psychology preferred.
