Lutheran Social Services of Wisconsin & Upper Michigan in Walworth County seeks a Care Coordinator for the CLTS Waiver Program. The role guides families to access services and navigate Medicaid-funded supports.

You will assess needs, develop service plans, coordinate with providers, document activities, and participate in team meetings. Remote work up to 2 days/week; daily community travel required.

Qualifications

• Bachelor’s degree in Social Work or related human services field.
• Minimum 1 year of experience working with children/youth with disabilities.
• Spanish preferred but not required.
• Proficient in computer systems and electronic health records (EHR).
• Strong interpersonal and organizational skills.
• Valid driver's license and reliable transportation (MVR check).

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our CLTS Waiver Program team in Walworth County!

The Children’s Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities. As a Care Coordinator, you’ll be the primary point of contact for families, helping them access and navigate essential services with compassion and care.

What You’ll Do
