Lutheran Social Services of Wisconsin & Upper Michigan is hiring a Bilingual Care Coordinator for the CLTS program in Milwaukee County. The role requires fluency in Spanish and English and involves assessing families, coordinating services, and developing care plans to help children stay safely at home and within the community.

The position is full-time with a hybrid work schedule (up to 2 remote days/week) and travel across Milwaukee County.

Qualifications

• Bachelor's degree in a human services-related field (Social Work, Psychology, Counseling, Sociology, etc.).
• At least one year of experience with children or individuals with developmental, physical, or behavioral health needs.
• Fluent in Spanish and English (required).
• Valid driver's license and ability to travel throughout Milwaukee County.

Now Hiring: Bilingual Care Coordinator – Children’s Long-Term Support (CLTS) Program

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a Bilingual Care Coordinator to join our growing Milwaukee County CLTS team. This role requires fluency in both Spanish and English.

The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid program that supports children with developmental, physical, and severe emotional disabilities. As a Care Coordinator, you will partner with families to assess needs, coordinate services, develop individualized care plans, and connect children with resources that help them remain safely in their homes and communities.

Schedule: Full-time, Monday-Friday, first shift
Work Environment: Hybrid schedule with up to 2 remote workdays per week

What You'll Do

• Complete assessments to identify client and family needs.
