Medica, a nonprofit health plan serving Minnesota and beyond, is seeking a Care Coordinated Products (CCP) Support Specialist to aid care coordination and documentation. The role handles authorizations, referrals, data entry, email inquiries, and cross-functional ad-hoc projects in an on-site office setting.

Candidates should have a high school diploma and 2+ years related experience; proficiency with Microsoft Office and EMR/EHR is preferred. The role is onsite in Minnetonka, approx.

Qualifications

• High school diploma or equivalent required.
• 2+ years of related work experience.
• Knowledge of CCP preferred.
• EMR/EHR experience.
• Proficient in Microsoft Office (Word, Outlook, Excel).
• Strong organizational skills with ability to manage competing priorities.

Responsibilities

• Managing care coordination and waiver assessment processes, including data entry.
