Salary ranges from $64,000 to $74,957.37 based on experience, education, and licensure.

Qualifications

• Bachelor’s or Master’s degree in social work, education, counseling, psychology, or related field preferred.
• LSW or LPC certification is required.
• Three years of experience providing coordinated supportive services or care coordination.

Responsabilités

• Supervise and oversee care coordination team members through meetings, supervision, and regular communications.
• Provide training for direct care coordinators in home and community settings.
• Ensure completion of required documentation and adherence to care plans, assessments, and outcomes reports.

Formation

Bachelor’s/Master’s degree in social work, education, counseling, psychology, or related field
