Job summary

Qualifications

• 5+ years in a healthcare-related field supporting patient care, public health, or population health.
• At least 2 years of supervisory experience leading teams.
• Strong organizational, time management, and communication skills.
• Proficiency in Microsoft Excel for data analysis and reporting.
• Experience or interest in underserved populations and community health.
• Valid driver's license and auto liability insurance.
• Ability to perform home visits and outreach to patients.
• Knowledge of local community resources and services.
