You will build relationships, educate patients, and collaborate with interdisciplinary teams to promote health and reduce hospitalizations, while navigating community needs with empathy and integrity.

Qualifications

• Minimum of 2 years in health-related work, e.g., CHW, peer support, or medical assistant.
• High school diploma or GED required.
• Valid driver’s license with reliable transportation for territory travel.
• Experience with chronic care management or chronically ill/elderly patients.
• Technologically proficient with basic computer skills and EMR systems.
• Bilingual/multilingual abilities welcomed.

Responsibilities

• Deliver chronic care management to high‑complexity patients, primarily at home.
• Guide enrollment, coordinate care, and facilitate virtual appointments.
