Job summary

Upward Health is seeking a Supervisor, Care Coordination to oversee the daily work of Care Specialists, including medical assistants, CNAs, and community health workers. The role focuses on supervising, coordinating patient care across providers, and supporting training and compliance initiatives.

The Supervisor will manage caseloads, conduct home visits, and engage in community outreach while ensuring alignment with Upward Health policies and quality standards.

Qualifications

• Minimum of 5 years in a healthcare-related field supporting patient care, public health, or population health.
• At least 2 years of supervisory experience.
• Strong organizational, time management, and communication skills.
• Proficiency in Microsoft Excel for data analysis and reporting.
• Experience or interest in working with underserved populations, particularly in community health.
• Valid driver's license and auto liability insurance.
• Ability to perform home visits and outreach.
• Knowledge of community resources and services.

Responsibilities

• Oversee day-to-day activities of Upward Health’s Care Specialists and supervise a multidisciplinary team.
• Support training efforts and ensure care coordination across medical and behavioral providers.
• Assist the Manager of Clinical Operations with leadership duties.
• Handle community outreach and manage caseloads.
• Ensure compliance with Upward Health policies and regulatory requirements.
