Job summary

The Care Specialist at Upward Health is a key member of our in-home care team, delivering chronic care management to high-complexity patients. This role predominantly works in patients' homes and communities, with some virtual visits, guiding enrollment and coordinating care plans.

Qualifications

Minimum 2 years of relevant work experience (e.g., CHW, Peer Support, Medical Assistant).
High school diploma or GED required.
Valid driver’s license and reliable transportation for territory travel.
Experience with chronic care management or chronically ill/elderly patients.
Proficient with basic computer skills and EMR systems.

Responsibilities

Deliver chronic care management primarily in patients' homes and communities (90%), with some virtual care (10%).
Engage patients, guide enrollment, and coordinate care with providers.
Build relationships, coordinate care, and educate patients to promote health and prevent hospitalizations.
Ensure adherence to care plans and effective communication within interdisciplinary teams.
