Job summary

CVS Health Corporation is seeking a Case Management Coordinator in Ohio’s West Region for a full-time field-based telework role. Travel within Western Ohio to member homes may reach up to 50% of the time, with hours Monday–Friday 8–5 and potential after-hours as needed.

You will assess member needs, coordinate care plans, and work with care managers, Medical Directors, and family teams to ensure timely access to services and quality outcomes. 40-hour work week with comprehensive benefits.

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, developmental disabilities, juvenile justice, or related public sector field.
• 2+ years experience in family systems, community resources, case management, or related areas.
• 2+ years with Ohio delivery systems and local networks.
• Must reside in the West Region of Ohio (Butler, Hamilton, Clermont, Warren, Montgomery, Allen, Hardin, Defiance, or Lucas).
• Willing and able to travel up to 50% within the region; mileage reimbursed; flexible hours.

Responsibilities

• Coordinate and implement assigned care plan activities and monitor progress.
• Collaborate with care managers, Medical Directors and other health programs to overcome barriers.
• Identify and escalate quality of care issues and coordinate with Child and Family Teams.
• Provide coaching and support to empower members in healthcare decisions.
• Serve as single point of contact for members to address care gaps.
