Job summary

CVS Health Corporation in Ohio seeks a Case Management Coordinator to help coordinate care for children and families within the OhioRISE program. The role requires evaluating member needs, coordinating care plans, and collaborating with health teams to improve outcomes.

The position is full-time, field-based telework with travel up to 50% and a typical 8–5 schedule, with flexibility for extended hours as needed.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office applications.
• 2+ years of experience in children’s mental health, child welfare, or related public sector fields.
• 2+ years in areas like family systems, case management, or counseling.
• 2+ years working with Ohio delivery systems and local networks.
• Willing and able to travel within the assigned region up to 50% of the time.

Responsibilities

• Be clinically and culturally competent in managing complex cases across child-serving systems.
• Evaluate referred members and coordinate and monitor care plan activities.
• Collaborate with care managers, Medical Directors, and other programs to overcome barriers and ensure optimal outcomes.
• Provide coaching and information to empower members to make informed health decisions.
• Serve as a single point of contact for members and address immediate gaps in care.
