Job summary

CVS Health is seeking a Case Management Coordinator in Ohio to support the OhioRISE program. You will coordinate care plans, connect families with services, and partner with clinical teams to improve member outcomes.

The role is full-time, field-based with telework options in Ohio, requiring up to 50% travel within the region. Strong collaboration, communication, and organizational skills are essential.

Qualifications

• 2+ years in behavioral health, social services, or human services
• 2+ years with MS Office Suite applications (Outlook, Word, Excel, SharePoint)
• 2+ years in children\'s mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services

Responsibilities

• Coordinate and monitor care plans for members.
• Collaborate with case managers, supervisors, Medical Directors and health programs.
• Provide coaching and support to empower members in healthcare decisions.
• Serve as a single point of contact for members and help remediate gaps in care.
