Job summary

Superior HealthPlan is seeking a Care Coordination team member to support outreach to members and assist in implementing care/service plans. The role involves coordinating with healthcare providers and community partners to ensure timely care, documentation, and education for members and families.

The ideal candidate will have healthcare administrative support experience, strong communication skills, and the ability to thrive in a fast-paced environment.

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.
• Excellent communication and attention to detail.
• Ability to adapt to a fast-paced, service-oriented healthcare environment.

Responsibilities

• Provides outreach to members via phone or home visits to engage members and discuss care/service plans.
• Coordinates care activities with providers, partners, and members to accommodate changes.
• Handles inquiries related to care plans and services.
• Communicates with care managers and practitioners to facilitate member services and continuity of care.
• Supports documenting and maintaining member records per state/regulatory requirements.
• Follows policies compliant with contractual and regulatory guidelines.
• Identifies needs and makes referrals to care managers and community resources.
• Provides education on benefits and resources available.
