SuperCare Health in California is seeking a Telecare Care Coach to support Chronic Care Management programs. The role blends in-office and remote work, with 2-3 days in the office, focusing on administrative tasks, scheduling, and patient outreach to ensure high-quality care delivery.

You will handle data entry, maintain patient records, coordinate with care teams, and support the Clinical Director while ensuring accurate documentation and responsive service for patients and providers.

Formación

• High School diploma required.
• Three (3) years’ experience in customer service or transaction processing in a Managed Care setting.
• Knowledge of Medicare Advantage and enrollment activities preferred.
• Proficient typing and MS Office (Word, Excel, Access).

Responsabilidades

• Enter new patient demographics into database and prepare kits for delivery.
• Schedule initial home visits and equipment delivery; maintain census.
• Verify information for accuracy and completeness; coordinate PCP/specialist/case management details.
• Maintain program information in electronic systems; manage equipment logs and returns.
• Support the Clinical Director by scheduling meetings and taking minutes.
• Schedule Interdisciplinary Care Team meetings and record minutes.
• Respond to inquiries from patients, families, and care team members.
• Perform outreach calls and set up appointments for in-person or video visits.
• Maintain/update required reporting and stay current on products and insurance trends.
