Talentify is seeking a Care Coordinator to assist the Case Management team with discharge planning coordination, transportation, and general department support.

Qualifications

• Four years of general office or secretarial experience or Associate’s degree or certificate in office occupations of office technology AND two years of general office or secretarial experience or Bachelor’s degree and two years of general office or secretarial experience or An equivalent combination of training and experience.

Responsibilities

• Provide discharge plan coordination support to the Case Management team.
• Assist with transportation and general departmental support.
• Communicate clearly with patients, families, and staff; document appropriately.
• Support staff with scheduling, data entry, and reporting tasks.
• Utilize Microsoft Office tools to prepare reports and presentations.
