This role involves leading care management activities, monitoring workflows, and ensuring compliance and timely patient transitions. You will assess patient plans, coordinate across disciplines, and communicate with families and payers while maintaining adherence to regulatory requirements.

• Provides leadership and direction for ongoing staff development.
• Rotates on-call responsibilities with other staff.
• Plans, implements, and enforces care management protocols and guidelines.
• Evaluates reimbursement, denials, and length of stay to improve delivery of care.
• Leads and coordinates care management staff to meet goals and ensure effective communication.
• Maintains timely and accurate discharge planning and transitions.
• Collaborates with interdisciplinary teams to optimize patient care and outcomes.
• Reviews patient status daily and communicates barriers to timely discharge.
• Establishes collaborative relationships with providers and revenue cycle teams.
• Performs miscellaneous duties and participates in process improvement meetings.
