Qualifications

• H.S. Diploma or GED required.
• Some college coursework preferred.
• 2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field required.

Responsibilities

• Provides department-related communications to internal and external customers.
• Assists utilization management processes to maintain compliance with federal, state and managed care guidelines.
• Supports transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
• Maintains data integrity.
• Provides professional, efficient and courteous service to the customer while maintaining confidentiality.
• Works effectively with peers, nursing and medical staff and maintains a positive attitude.
• Responds to requests personally, tactfully, and courteously.
• Relays messages to appropriate individuals efficiently and in a timely manner.
• Promotes a positive work environment, and contributes to the overall team effort.
