Qualifications

• Excellent communication skills, both verbal and written.
• Effective critical thinking skills and ability to anticipate patient discharge needs.
• Moderate computer skills.
• Effective decision‑making/problem‑solving skills, demonstrated creativity in problem solving.
• Bilingual (Spanish).

Responsibilities

• Supports Social Work activities, including coordinating transportation vouchers and amenities.
• Reviews and routes third‑party payer requests for concurrent review and notifications.
• Assists with post‑discharge planning, including housing resources and equipment coordination.
• Maintains complete and accurate patient resource materials and schedules care conferences.
• Answers Social Work and Utilization Management lines and routes inquiries to appropriate staff.
• Documents status and actions in the medical record and ensures timely follow up.
