Talentify is seeking a Discharge Planner to provide direct and indirect client care, including psychotherapeutic services, within a psychiatric hospital setting in Louisville, KY. The role integrates with an interdisciplinary team and serves as the primary liaison for discharge planning and continuum of care.

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• One year post-degree experience preferred.
• Ability to perform assignments with minimal supervision; work under stressful conditions.
• Effective reading, writing, and verbal communication skills.

Responsabilités

• Provides direct and indirect client care and psychotherapeutic services.
• Acts as liaison between facility and outside agencies for discharge planning.
• Facilitates referrals and collects necessary clinical materials for referrals.
• Coordinates travel and discharge appointments; communicates updates to therapy team.
• Maintains case documentation and timely discharge planning notes (within 24 hours).
• Participates in community activities to build relationships with providers.
• Collaborates with Utilization Review and clinical teams; conducts weekly referrals to providers.
