Job summary

CVS Health Corporation is seeking a full-time Field-based Case Management Coordinator in Ohio to support the OhioRISE program. You will travel within Central and Southeast Ohio to member homes and locations, coordinating care plans and engaging families to improve outcomes.

The role emphasizes clinical coordination, empowerment of members, and collaboration with multiple health teams to ensure timely access to services and quality care in the community.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office Suite applications.

Responsibilities

• Be clinically and culturally competent with training to manage complex cases in the community across child-serving systems.
• Coordinate and implement care plan activities and monitor progress.
• Provide coaching, information, and support to empower members to make informed health decisions.
