Job summary

CVS Health Corporation is seeking a Field-Based Case Management Coordinator in Ohio to support the OhioRISE program. The role requires coordinating care for children and families, traveling across central and southeastern Ohio as needed, and working a Monday–Friday schedule with extended hours to meet member needs.

The ideal candidate has 2+ years in behavioral health or related public sector work, plus a bachelor’s degree or equivalent master’s level training.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related public sector field.
• Willing and able to travel within Ohio region up to 50%.
• Bachelor's degree or non-licensed master level clinician; must reside in Ohio.

Responsibilities

• Provide case management, care coordination, and education for members using care management tools.
• Collaborate with OhioRISE program and health partners to support member outcomes.
• Evaluate member needs, eligibility, and coordinate care plan activities.
• Serve as a single point of contact and address gaps in care.
