Job summary

South Shore Health System is seeking a Clinical Social Worker to advocate for patients, coordinate services, and support transitions from hospital to home. The role collaborates with RN Case Managers and an interdisciplinary team to optimize outcomes and reduce costs.

The on-site position requires 4–5 days per week with some holidays; no nights, weekends, or on-call hours. Licensure as LICSW or LMHC is required or possible.

Qualifications

• Masters degree in social work required, independent licensure preferred.
• Excellent communication skills and ability to work autonomously within a multidisciplinary team.
• Experience with pediatric, elder, and diverse community populations is desirable.

Responsibilities

• Respond to and prioritize urgent high-risk cases and coordinate referrals (financial, educational, counseling).
• Complete and document psychosocial assessments within required timeframes in EMR.
• Participate in CMS Teams, IDRs, and discharge planning to ensure timely transitions.
