South Shore Health System is seeking a Clinical Social Worker to advocate for SSH & SSMC clients, coordinate services with the RN Case Manager, and support transitional care planning to improve outcomes.

You will work with a multidisciplinary team to optimize care coordination, address behavioral health needs, and guide patients and families through discharge and community reintegration, with on-site presence 4-5 days/week.

Qualifikationen

• Master's degree in Social Work and independent licensure required.
• Experience with pediatric, elders, and diverse populations preferred.
• Healthcare experience required.

Aufgaben

• Respond to high-risk referrals and coordinate community resources.
• Perform psychosocial assessments within 24 hours of admission or referral.
• Participate in CMS Teams and discharge planning.
• Collaborate with multidisciplinary teams to ensure timely discharge.
