Job summary

South Shore Health System is seeking a Clinical Social Worker to advocate for patients, coordinate services with the RN Case Manager, and facilitate transition planning to home or community after discharge. The role involves working with a multidisciplinary team to optimize care coordination, behavioral health, and wellness across admissions, ED visits, and post-discharge stages.

The position requires licensure (LICSW or LMHC), a Master’s in Social Work, and strong communication skills.

Qualifications

• Master's degree required.
• Must be independently licensed (LICSW or LMHC).
• Experience with pediatric, elder, and diverse community populations preferred.
• Proficiency in EMR documentation; EPIC and Outlook preferred.

Responsibilities

• Respond to and prioritize urgent high-risk cases and coordinate community referrals.
• Review patient lists and settings to identify those needing social work interventions.
• Participate in interdisciplinary rounds and communicate psychosocial barriers and plan of care.
• Coordinate with care team and external agencies to ensure timely discharge and post-acute care.
