Job summary

South Shore Health System seeks an on-site Clinical Social Worker to advocate for patients, coordinate with the RN Case Manager, and manage transitional care planning. You will collaborate with a multidisciplinary team to optimize care, monitor outcomes, and facilitate safe transitions home or to the community after discharge.

The role requires Master’s in Social Work, independent licensure (LICSW or LMHC), and strong communication.

Qualifications

• Master's degree required and independently licensed.
• Experience in healthcare social work required.
• Experience with pediatric, elder, and community populations desirable.

Responsibilities

• Respond to high-risk cases including crisis interventions and referrals.
• Coordinate with CMS Teams and community resources for patient discharge planning.
• Document psychosocial assessments within EMR within 24 hours of admission or referral.
• Participate in interdisciplinary rounds and care conferences to support timely discharge.
