South Shore Health System seeks a Clinical Social Worker to advocate for patients and coordinate services with the care team. You will support transitions, manage psychosocial assessments, and collaborate on post-discharge planning to ensure quality, cost-effective outcomes.

The role emphasizes on-site presence 4-5 days a week with no night, weekend, or on-call hours required, working within a multidisciplinary team to support patients and families.

Qualifications

• Master's in Social Work required; LICSW or LMHC independently licensed.
• Experience with pediatric and elder populations preferred.
• Excellent communication and ability to work autonomously within a multidisciplinary team.

Responsabilités

• Coordinate with the RN Case Manager and multidisciplinary team for care transitions.
• Complete psychosocial assessments within 24 hours of admission or referral.
• Assist with discharge planning and transition to home or community services.
