Job summary

Qualifications

• Master's degree in Social Work and state licensure (LICSW).
• CPI (Crisis Prevention) within 1 month of new position and renew annually.
• Ability to read, write and communicate in English.

Responsibilities

• Plans, organizes and implements social work services to patients and families.
• Plans for discharge and after-care; maintains contact with referring agencies and community resources.
• Assists patients and families to adjust to illness and hospitalization; conducts comprehensive psychosocial assessments.
• Coordinates with treatment team to create discharge plans incorporating psychosocial needs.
• Maintains communication with health care team including emergency department psychiatric team and collateral community resources.
