South Shore Health System is seeking a Clinical Social Worker to advocate for patients and coordinate services with the RN Case Manager and care team. The role focuses on optimizing transitions from hospital to home and community, with emphasis on behavioral health and wellness.

On-site role 4-5 days a week, no nights or weekend shifts, and no on-call requirements, ensuring comprehensive support for patients across pediatrics to geriatrics.

Competenze

• Master's degree in social work and independent licensure required.
• Healthcare experience, especially with pediatric, elder, and community populations, is desirable.
• Proficiency with EPIC EMR and Microsoft Outlook preferred.

Mansioni

• Advocate for patients and coordinate services for discharge planning.
• Collaborate with RN Case Manager and interdisciplinary staff to optimize care.
• Complete psychosocial assessments within 24 hours of admission or referral.
• Prioritize high-risk cases and coordinate community referrals.
• Participate in CMS Teams, discharge planning, and care transitions.
• Escalate issues to Case Management leadership as needed.
• Support regulatory compliance and timely documentation.
