Job summary

Choice Health at Home in Reno, NV is seeking a Full-Time Patient Services Coordinator to schedule patient services and work with staff across disciplines. The role may cross-train as the Medical Records Specialist (MRS) and requires coordination with nurses, patients, and referral sources.

The ideal candidate has a high school diploma, LVN/LPN training preferred, and a state license (NLC multi-state preferred).

Qualifications

• High School diploma required; LVN/LPN preferred.
• Current state LVN/LPN license; multi-state NLC license preferred.
• CPR certification required if patient visits occur.
• 1 year of homecare experience preferred.
• Reliable transportation and auto liability insurance.

Responsibilities

• Complete workflow tasks in the Agency’s EMR as PSC.
• Prioritize scheduling of new patient admissions with Admin or RN supervisor.
• Coordinate care with nurses, patients, and other health professionals.
• Provide clear communication to patients, staff, and referral sources.
• Respond to Agency and patient needs professionally and creatively.
• Demonstrate commitment, professional growth, and competency.
• Participate in Agency-sponsored in-service training.
• Participate in the Agency’s QAPI program, as directed.
• If clinical, perform patient visits and complete documentation timely.
• Promote the Agency philosophy and policies to ensure quality care.
• Cross-train as the Medical Records Specialist (MRS) as needed.
