Visiting Nurse & Hospice of Litchfield County is seeking a full-time Clinical Team Assistant/Scheduler to join our Home Health & Hospice team. You will coordinate patient visits, support clinicians, and help ensure timely, high-quality care across our service area.

The ideal candidate is highly organized, with excellent communication and time-management skills, comfortable in a fast-paced environment, and proficient with computer systems to maintain accurate scheduling in the EMR.

Qualifications

• Healthcare or scheduling experience preferred.
• Excellent organizational and communication skills.
• Ability to manage multiple priorities.
• Comfortable in a fast-paced environment with schedule changes.
• Strong computer skills and ability to learn new systems.
• Independent yet collaborative team member.

Responsibilities

• Coordinate daily schedules for nurses and other staff.
• Schedule admissions, routine visits, and follow-ups.
• Communicate schedule changes with patients, families, clinicians, and leadership.
• Monitor schedules and adjust as needs arise.
• Assist with weekend, holiday, and on-call coverage.
• Maintain scheduling in the EMR.
• Track missed, cancelled, and rescheduled visits.
• Assist with referrals and onboarding of new patients.
• Provide professional customer service to patients and staff.
